Good idea about writing a letter - that way you can get all the facts in and he will have the letter around to remind him of a better way to proceed with future patients. This is just my opinion, but if you can possibly find some way to set a positive tone to the letter, (or find some minor thing to include as a complement) he may be more likely to accept a suggestion for a different way to handle a situation like yours that caused you so much stress and worry).
