I had the weekly infusion at the James Cancer Hospital at Ohio State University. It was explained to me that there testing showed a slight increase in efficacy over the larger doses, and a decrease in morbidity. He said the efficacy increase was small, but certainly no less effective than the larger doses. Thus, the decrease in morbidity prompts them to use the weekly in most cases.
