Exposed bone is a logical place for entry of bacterial infection, maybe even osteomylitis (which is a bacterial infection in the bone marrow). Have they tried a round of antibiotics yet? It could be ORN (OsteoRadiaNecrosis) as well.
Bone infections are areas of concern. I had ostemylitis as a child and it was very serious. I was bedridden for almost a year.
