I checked the NYC subway map at http://www.mta.nyc.ny.us/nyct/service/threelin.htm, and this appears to be the best bet. The Seventh Ave. Express goes from Penn Station and stops at 135th Street and Lenox Ave. Not sure exactly how many avenues over Lenox is from 5th Ave. There are also 3 other trains that stop at 135th Street. Not sure if any others are closer.
