Show him the exact area and ask for biopsy. Tell him this had gone on long enough and you want it figured out. Plus a biopsy will ease your mind once you have a definitive result. Also how is it he thinks you may have an infected submandibular gland yet can't palpate it? Usually they swell if infected... good luck and take care.
