Dx SCC base of tongue, T2N2M0, Feb 05. L neck disection with some neuro/muscular loss to shoulder, Chemo, 37 IMRT, PEG. All tolerated well. NPO from second week IMRT untill Seventh weekpost radiation, lost 20#. Prior to radiation I had all dental work done including extraction of wisdom teeth. Was doing well and cleared for IMRT by oral surgeon. 10 weeks post radiation I have kicked out a bone chip and now have a hole in my gums, about 1/16 in in dia. that doesn't want to heal after 4 weeks. Got reassurance from Radiation oncologist and oral surgeon and told to give it time. Am on soft diet with intense oral hygene. I don't want hyperbaric 02. Any insight out there? EDD | | |

This is sometimes a 50/50 thing. It's a function of how much of the micro-vascularization of the area was damaged during treatments. 02 will speed that up and help this thing heal right up. If it is a flat surface of exposed bone there are no additional issues. If it is a bone spur or little spike, you want to have the oral surgeon take a pair of bone clippers and snip it off so that the shape itself is not an obstacle to healing.
