Shots touching the higher (smaller) rings are given that count. Matches are usually limited to a set number of rounds (shots) fired. In normal targets the highest center area is worth 10. Therefore a perfect score would be all shots inside that ring. 100 if your match calls for 10 shots.
There are some fun timed matches that use different targets like bowling pins that must be knocked down, in those matches you have no restriction on the number of shots but instead are timed until you knock them all down.
An interesting twist might be to fire shotguns at those pesky cancer cells to simulate radiotherapy :p
