Tumors place thousands of cancer cells into the bloodstream every day. The immune system does a good job of taking them out usually. Surgery shouldn't make a difference in metastesis risk.

IMRT can be repeated if not in the same location. They always give the minimum dose of 60-66 cGy's of radiation up to around 70 cGy.
