I had the mouthpiece and had to give it up for the last few (5) treatments. I just couldn't get my mouth open enough to put it in, and when I did it gagged me which was not going to work being under that mask.
Looking back the worst experience I had with the mouth piece was one Monday after one of the tech's had it soaking in Listerine all weekend to kill the germs.... Whoa buddy, talk about burn!
