It happens when the muscles on the sugery side tighten up. The more I try to stretch them the worse the pain gets. The no one seems to know if this is because of the surgery or the radiation. My Dr. has given me a muscle relaxer which I take when this happens and that does the trick.
In addition I had significant shoulder pain post surgery, because my shoulder is not as well supported. This is common after a radical neck dissection when the nerve is removed. Fortunately I have built up the other muscles in that area and have less problems now.
You didn't mention if Jim had surgery or not so I'm not sure if this will be helpful or not. In my opinion PAIN = DOCTOR visit to have it checked.

Mark, 21 Year survivor, SCC right tonsil, 3 nodes positive, one with extra-capsular spread. I never asked what stage (would have scared me anyway) Right side tonsillectomy, radical neck dissection right side, maximum radiation to both sides, no chemo, no PEG, age 40 when diagnosed.
