Hubby had to have all teeth out Pre treatment due to radiation treatments. Submitted bill to medicare myself because the OSU dental college is not a medicare provider. Put in there a good explanation of why i was submitting etc.and had letter from facility stating medical necessity. Medicare sent me a letter and asked for two things

1. Description of the service funished or the cpt/hcpcs code

2. Diagnosis code

Can any help with this?
