What I did was make a spreadsheet on my computer with all the doctors" names, phone numbers, names of nurses or secretaries in case I needed to call, and places for medical procedures, history, bills paid or still pending. I had another one for all the different medications, schedules for when and how often taken, records of foods/liquids taken as well as the outgo of the patient. (Whatever comes out of the patient's body). I had a special place for notes and reminders too. With your own spreadsheet, you can taylor it to your specific needs or situation.

A huge help was to get the plastic sheets (3-hole punched for a notebook) that hold multiple business cards and I picked up a card for each care provider while in the office. There is a tab for medical reports, one for the CCC he sees/saw during his treatment and now for followups. There is a tab for insurance information..so on and so on.
I just found that doing this gave me a sense of control in a very chaotic time and helped when we had to fill our paperwork in all the doctor's offices (I carried that notebook a bunch!) I did not have to depend on my memory for all the little details.

Deb..caregiver to husband, age 63 at diagnosis, former smoker who quit in 1997. DIAGNOSIS: 6/26/07 SCC right tonsil/BOT T4N0M0 TREATMENT START: 8/9/07 cisplatin/taxol X 7..IMRT twice daily X 31.5. TREATMENT END: 10/1/07 PEG OUT: 1/08 PORT OUT: 4/09 FOLLOWUP: Now only annual exams. ALL CLEAR!

Now I am just trying to get everything typed into a file and I did find a fill in the blank Medical Record Care Book online, i am just starting to work on it now.
