You fill it out and have the doctor sign it.

I recommend getting the license plate and a placard - the cops scrutinize the plates a whole lot less then the placard (probably because it's easier to abuse the placard). The DMV here has a special line for disabled so the wait wasn't very long. Some HMO's have the DMV forms there as well or you may be able to download them online.
In California you can park free at meters for an unlimited time, some concert venues will put you in the VIP parking for free, etc.
